FFN Waiver Agreement
Minor Volunteer Participation Form
- Student volunteers age 9 and up are welcome! All volunteers under 18 must be accompanied and supervised by a parent/legal guardian. If a parent cannot attend, a Minor Volunteer Participation Form is required.
  - Minors 9-15 must attend with a designated adult and have a signed form.
  - Minors 16-17 can attend without a parent but need a signed form.
